Collapse: Neurological

Status: Reserved slug — full retrograde essay in development.

This is the second panel of the Terminal Event Diptych.

The Retrograde Chain

Cognitive decline → Neuronal loss → Neuroinflammation →

Microglial activation → Blood-brain barrier compromise →

Circulating bEVs → Periodontal pocket

Key Mechanisms (to be developed)

  • bEV transcytosis across brain endothelium
  • Gingipain-mediated tau phosphorylation
  • LPS-driven microglial priming via TLR4
  • NLRP3 inflammasome–dependent IL-1β release in hippocampal tissue
  • bEV-nucleated protein aggregation at amyloid-periodontal interface

Pedagogical Method

Terminal Event Pedagogy — start with the

family noticing word-finding difficulty, work backward. The clinician already

knows what Alzheimer's is. They need to see the path from pocket to

hippocampus.

The Diptych Argument

This essay pairs with Collapse: Cardiovascular.

Same mechanism, different organ system. The conservation of the

bEV cascade across cardiovascular and neurological

endpoints is the argument — not two diseases, one transport program.
